AMC Health to Provide Remote Patient Monitoring Services to Crystal Run Healthcare

Crystal Run Healthcare (Crystal Run), a NY-based multispecialty group with more than 300 providers that is one of the fastest growing physician groups in the country, announced that it has entered into a partnership agreement with AMC Health to improve care coordination and outcomes for patients with chronic conditions.

Under the agreement, AMC Health will provide solutions that will enhance care for Crystal Run patients with heart failure, diabetes, chronic obstructive pulmonary disease and other chronic illnesses. The company also will provide remote patient monitoring services that will enable Crystal Run to eliminate and avoid gaps in care that occur after patients are discharged from a hospital.

"We selected AMC Health because their proven and scalable telehealth solutions significantly reduce unnecessary resource utilization, particularly preventable 30-day readmissions," said Jonathan Nasser, MD, co-chief clinical transformation officer at Crystal Run Healthcare. "By identifying care gaps and providing services and tools that support seamless and effective care transitions, AMC Health will make it easier for our doctors to deliver high-quality, cost-effective and efficient evidence-based care. This will position us to successfully navigate the shift to an outcomes-based reimbursement system."

AMC Health will provide a post-hospital discharge service and tailored disease-specific programs employing biometric monitoring, healthcare coaching and logistical services for in-home monitoring.
