Healthx now offers a new remote patient-monitoring platform to improve outcomes for chronically ill members with diabetes, hypertension, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD) and multiple co-morbidities.
According to the Centers for Disease Control and Prevention (CDC), more than 75% of health care costs are due to chronic conditions. The service is called mHealth Manager and utilizes an FDA approved biometric data collection platform that will support care managers at health insurance companies, providing near real-time data for high-risk plan members.
By using wireless transmission devices for monitoring blood sugar levels, blood pressure, weight and pulse oximetry, the mHealth Manager platform will equip care managers with information necessary to reduce emergency room visits and hospital admissions, and stabilize health conditions.
mHealth Manager works by having the patient take his or her biometric reading at home. The reading is transmitted wirelessly to the care manager and displayed almost immediately on an online dashboard. As high-risk health plan members track their health data via a home PC or mobile device, mHealth Manager facilitates precise and timely interventions between care managers and health plan members whose care they are monitoring.
