ALR Technologies Inc., a medical device company providing remote monitoring and care facilitation for patients with chronic diseases, today released a video demonstrating how the ALRT's Diabetes Care Facilitators (DCFs) use the Health-e-Connect System.
DCFs monitor patient blood glucose data, testing and care plan adherence as well as facilitate care using a robust communications platform to connect patients, providers, caregivers and health plans. Finally, the video also explains how the Health-e-Connect system is designed so DCFs can monitor patient activities that are important for health plans to achieve optimal NCQA HEDIS scores.
